If you or any of your group members are participating in multiple items, please name your multiple entries where participants are participating in multiple entries, otherwise this form will be treated as incomplete because it creates scheduling conflicts. This will also enable the participants to prepare for multiple items in a timely manner.

Two CDs clearly marked w/item & Choreographer's name as registered in the entry form (No audio tapes please) should be delivered to Cultural Program Committee, by Friday, September 24th by 6 pm at The Richmond Convention Center. Please test your CDs on regular CD players. Computer and MP3 CDs format may not work with the Audio system at the Convention Center. Only one track per CD, per performance PLEASE!!
